EXPLICIT CONSENT TEXT OF THE ATHLETE AND/OR THE ATHLETE'S PARENT

As Max Potential Sportif Yatırımlar A.Ş. (“Max Potential” or “Company”), we request your explicit consent regarding the following matters within the scope of the processing and/or transfer of personal data of athletes within the scope of the Personal Data Protection legislation:

In case of application for the basketball camp and acceptance of the application, the personal data I have transferred during the camp process (my identity information, personal information, experience, education, CV, professional experience information) will be stored for 2 years in order to be able to propose a possible partnership in the future and I will be contacted by phone, e-mail and message regarding this matter.

If your application is submitted and/or accepted, your identity, visual and audio data will be transferred to the domestic or international systems of the platform service provider within the scope of the use of video recording during the camp,

During the evaluation phase of your application, an online interview will be conducted and your identity, visual and audio data will be transferred to the domestic or international systems of the platform service provider during the online meetings held via Google Meet or Zoom platform,

If your application is accepted, all my personal data (including my photo, audio and video recordings) obtained during the camp process will be stored for 2 years in order to be able to propose a possible future partnership, shared with third party people and/or companies that Max Potential works with, and used on Max Potential social media and communication platforms.

I accept and declare that I APPROVE WITH EXPLICIT CONSENT, as an athlete and/or athlete parent, to the processing and transfer of the athlete's personal data in line with the purposes stated above and in the information text, in accordance with the Personal Data Protection Law No. 6698.
